In order to ensure the security of your personal data and protect privacy, which is our primary concern, and respecting applicable law, MediSpace Sp. z o.o. has established a policy defining the principles for collecting, processing and using personal data.

Personal Data

Before using our website services and providing any data, please read the entire confidentiality policy. MediSpace Sp. z o.o. informs that all contact/address details received through the website will be used exclusively by MediSpace Sp. z o.o. for the following purposes:

– medical services management

– ongoing customer service,

– conducting preventive and health-promoting activities

– educational and training purposes

– statistical purposes

– marketing purposes (only for own products and services)

In each case, providing personal data in contact forms on the Company’s website is voluntary, and their processing takes place only with the user’s consent.

Based on the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation), we inform you that you have the right to:

– access to data (Art. 15),
– rectification and completion of personal data (Art. 16),
– be forgotten (Art. 17),
– request restriction of processing (Art. 18),
– data portability (Art. 20),
– object to data processing (Art. 21),

All data can be changed at the reception desk at MediSpace headquarters at Spokojna 5 Street in Warsaw. Personal data changes can only be made by an authorized MediSpace Sp. z o.o. employee and only with the Client’s authorization of changes (handwritten signature on the personal data change form)

Personal Data Administrator

In accordance with Art. 24(1) of the Act of August 29, 1997, on the protection of personal data, we inform that the administrator of your personal data is MediSpace Sp. z o.o. with headquarters in Warsaw at Spokojna 5 Street (01-044).

In accordance with Article 13(1) of the General Data Protection Regulation (GDPR), we inform that:

1) the administrator of personal data of Patients is MediSpace sp. z o.o. in Warsaw , address: Spokojna 5 Street, 01-044 Warsaw ;

2) the administrator has appointed a Data Protection Officer, whom you can contact regarding the processing of your personal data via email: inspektor.danych@medispace.pl;

3) the administrator will process your personal data based on Art. 9(2)(h) of GDPR, i.e., for the purpose of providing healthcare, which follows from the Act of November 6, 2008, on Patient Rights and the Patient Rights Ombudsman and the Act of April 15, 2011, on Medical Activity;

4) personal data may be shared with other authorized entities, based on legal provisions, as well as other entities with whom the administrator has entered into an agreement in connection with the provision of services for the administrator (e.g., external laboratory, law firm, software provider, external auditor, contractor providing personal data protection services);

5) the administrator does not intend to transfer your personal data to a third country or international organization;

6) you have the right to obtain a copy of your personal data at the administrator’s headquarters.

Additionally, in accordance with Article 13(2) of GDPR, we inform that:

1) Your personal data will be stored for the period resulting from legal provisions, i.e., from Article 29 of the Act of November 6, 2008, on Patient Rights and the Patient Rights Ombudsman ;

2) you have the right to access your data, rectify it or restrict its processing, as well as the right to file a complaint with the supervisory authority;

3) providing personal data is voluntary but necessary to achieve the above-mentioned purposes. The consequence of not providing data will be the refusal to provide healthcare services;
4) the administrator does not make automated decisions based on your personal data.

Information Collection

Using the website and web pages of MediSpace Sp. z o.o. means accepting the terms of this privacy policy. Each time you provide information through this website, you consent to the collection, use, and disclosure of this information in accordance with this privacy policy. Personal data is protected in accordance with the provisions of the Personal Data Protection Act. The Company collects your data only to the extent necessary for professional service operation.

Security Policy

The website and web pages are equipped with security measures designed to protect data under our control from loss, misuse, or modification. We are obligated to protect all information disclosed to us by Clients in accordance with security protection and confidentiality standards.

Access rights to users’ personal data have been limited to a minimum and are only available to selected Company employees through an authorized access system that monitors traffic in this regard in accordance with the Personal Data Protection Act.

When you visit our website or web service, data regarding your basic activities (IP address, browser type, operating system type) will be automatically recorded. This data will be used only for administrative and statistical purposes.

Communication with our servers is encrypted and uses SSL (Secure Socket Layer) protocol. The servers where personal data is stored are owned and controlled by MediSpes Sp. z o.o.

Newsletters, Other Forms, Surveys

The website and web pages offer users the option to unsubscribe from ordered newsletters. We provide Users with the option to remove their information from our database and enable them to stop receiving information sent via email. This can be done by following the instructions attached to the content of our messages. Users have the ability to ask questions or contact MediSpace Sp. z o.o. employees and register medical appointments. The Company stores correspondence with website users for statistical purposes and to analyze and improve service standards. Additionally, we inform that based on Art. 10(2) of the Act of July 18, 2002, on Providing Services by Electronic Means (Journal of Laws of 2002, No. 144, item 1204, as amended), providing an email address indicates consent to receive commercial information via electronic means of communication from MediSpes Sp. z o.o. within the meaning of Art. 2(2) of the above-mentioned Act.

Cookies

Each web browser can store text files (“cookies”) on the user’s computer disk. Cookies contain information necessary for the proper functioning of the website and web pages. The content of “cookies” does not allow for user identification. Personal data is neither processed nor stored through cookies. Cookies are stored on users’ computers for the purpose of:

  • maintaining user session (after login);
  • better adapting the service to user needs (e.g., remembering recently used connections to prepare appropriate suggestions).

It is possible to configure the web browser to completely disable the ability to store cookies on the computer’s hard drive. The effect of such a change will be the loss of ability to use the website and web pages.
